To submit these forms online, you MUST have a log-in. Your log-in will be the same log-in as your student e-mail log-in. To log-in, click on the "Sign in" link located in the upper right corner of the screen:
Make sure to type in "centstu\" before your username - see example below:
Click the "OK" button once you have typed in your User name and Password. After successfully logging in, you will notice "Welcome Your Name" displayed in the upper right hand corner of the page. See example below:
